Health + Wellness Programs

These programs can be specifically designed to address current health issues.  We are also able to tailor the education session/s to support community groups who wish to have a short presentation at their meetings and/or social events.  We are able to explain the benefits of exercise for their chosen topic, discuss current trends within health and fitness and/or discuss current research supporting exercise as a therapy to manage many conditions.

Injury Management and Manual Handling Education + Training

We can provide Early Intervention, Injury Prevention and Manual Handling seminars/education tailored to specific industry environments (e.g. childcare, construction, banking, general office etc).  We run sessions for a minimum of 5 participants to a maximum of 10 participants as most of our sessions often involve practical components.

Type 2 Diabetes Management Program (Medicare Allied Health Group Services Scheme)

The Merendi Type 2 Diabetes Management Program has been designed to enable participants to make positive changes in their current lifestyle to better manage their condition. Whether participants are newly diagnosed with Type 2 Diabetes or they require a refresher, our program will give them the tools and knowledge they need to take control of their Diabetes – and take control of their life. 

We offer a multidisciplinary approach to educating our clients with Type 2 Diabetes. Our program encourages self-management as well as having access to support and guidance from our experienced allied health team. Our program enables participants to better understand how to implement small, but positive lifestyle changes to their daily activities and to give them confidence that these changes will become part of their regular routine. 

Our program is facilitated by accredited dieticians and exercise physiologists and covers a combination of educational and practical components designed to help clients understand:

Our program format can be modified and changed to suit the audience. We cater for the indigenous, culturally and linguistically diverse (CALD) communities and other groups with specific needs (i.e. pre-diabetics/impaired glucose tolerance, Type I Diabetes, Gestational or newly diagnosed with diabetes). Our program format can also target specific aspects of diabetes management or for different stages of the diabetes continuum (i.e. weight loss, managing co-morbidities). 

Individual Assessment 

All participants will be assessed before commencing our group program. This includes taking a comprehensive patient history, identifying individual goals and preparing the client for an appropriate group program. 

Duration:  

Our program runs for 8 hours.

The program can run over a period of weeks or months depending on participants availability. 

Program Content: 

4 x Group Education sessions (1 hour each)

4 x Group Exercise sessions (1 hour each) 

Cost of Program: 

The total cost of the program is $250. This cost inclusive of all professional fees (Exercise Physiologist, Dietitian, Education sessions & Exercise classes). 

Eligible participants may be able to claim a Medicare rebate of up to $195.70 for this program. 

Pensioners with a Medicare Allied Health Group Services Scheme referral will be bulk billed.

"HEAL" Lifestyle Modification Program

 

 

 

 

Healthy Eating, Activity and Lifestyle (HEAL™)Program – HEAL™ is an eight week nutrition and physical activity program that primarily targets adults who are at risk of developing lifestyle diseases, such as cardiovascular disease and type 2 diabetes. Participants undertake an initial assessment, then join HEAL™ at any week in the cycle, continuing for eight consecutive weeks. The program includes a one hour group based exercise session accompanied by one hour of education each week, with participants reviewed upon program completion and again at 5 and 12 months. Individuals who participate in the HEAL™ program are also provided with a basic home-based exercise program for exercise uptake in between sessions.

HEAL is an accredited Lifestyle Modification Program under the Australian Government Prevention of Type 2 Diabetes program.

HEAL is a subsidised program that supports eligible people to prevent or delay the onset of chronic diseases and also manage their chronic disease. 

The subsidised program is available to people over the age of 18. 

Who can participate in HEAL™?
The program assists people who are overweight, or obese, with existing chronic disease or at risk of developing chronic disease to improve their health and well-being by providing lifestyle education with a focus on improving nutrition and physical activity and promoting self-management. Persons at risk of developing, or with existing diabetes, heart diseases or other conditions are examples of some of the participants that can attend HEAL™.
 
How do I refer my patients to HEAL™?
Patients can be referred to the HEAL™ program for Type 2 diabetes group services. This involves an individual assessment by the Exercise Physiologist or Dietitian, followed by 8 group exercise/education classes. To be eligible, patients must have Type 2 Diabetes and a GP Management Plan. These group sessions are in addition to the 5 individual allied health services available under Medicare.
